Regarding display index:
Output 1 and 2 default to display-index 1
You need to change output 2 to display-index 2 (as you discovered)
Output 3-6 are actually fixed to display-index 3-6...

PixelMixes only work from output 1... and have to be visible - so may not work offscreen...
So a DVI Parrot on output 1 for the pixel-mix would solve that issue - and move the projection to output...
Assuming that you are using a separate mix window for the Pixelmad, simply patch the 'Mix Intensity' (fade out) to a single DMX channel and it should give you overall intensity control of the Mix...

For reference - Shutter open/close and Lamp ON/OFF is possible via RS232 (admittedly not direct HTP) and is can be quicker than HTP - although you would need a serial server / RS232...

I started writing specific help sheets based on questions I get asked - it is a bit easier than writing a full guide or manual.
Re catalyst location - doesnt matter where it is.
But it makes life...
A few comments without going in to massive detail:

Not a good idea to keep catalyst in the applications folder amongst everything else.
Better to be in its own sub-folder - then it is...
Dont think so - it works fine for me on Snow Leopard 10.6.8
Do you still have 2 graphics cards in your system? - suggest removing one...

Not true -
We are purchasing new macs which are sold with Lion pre-installed. We then wipe the HD and re-install Snow Leopard via Disk Image (10.6.8)...
We leave the Lion rescue partition...
This is usually due to the location of your show files
Best to create a new folder on your desktop and copy the show files in to it.
Always work within a folder - since catalyst creates settings...
... theres just more stuff to turn off in lion
(cats have 9 lives...)
have you actually tried running any content though?...
Mac-Mini graphics cards aren't great... A single output is ok (1920x1080) or 2 lower resolution (2x 1024x768) - but much more than this...

but do watch out for change in mix window...
Just tested reducing mixes and the pixel-mad window doesn't change...
So your only option at the moment is to move the pixelmad window to the very top of the screen ad expand it as much as...
I assume you restarted after changing settings...
I think the only other way is to change screen resolution so you can see more window.
What resolution are you using?

Might help if you turn off some mixes ( and layers - unless you need all of them) in preferences in the catalyst menu- then there will be less mix buttons and you should be able to see the DMX...
